Dalesio and Associates, Inc. Referral Process
Please make sure when adding behavioral supports to plans that you add Dalesio and Associates associated with the following address: 6 Dickinson Drive, Suite 212, Chadds Ford (Service Location 0009).
- Support Coordinator(SC) begins process by emailing referral to admin@dalesiosupports.com or calling 610-363-0380. Support Coordinator should send ISP, location of client, and behavioral needs. Should more information be needed Dalesio and Associates Administrator will send additional document to be filled out by the SC.
- Dalesio and Associates Administrator will review the case and determine if additional information is needed.
- Dalesio and Associates will decide to serve client in a timely manner upon receiving referral and/or once all referral information is received.
- Once Dalesio and Associates makes a decision to serve the client, a consultant will be assigned in a timely manner. If the client chooses Dalesio for their behavioral support provider, the following steps should be followed.
*Dalesio and Associates administrator will contact the SC and inform the SC that a consultant was assigned. The consultant will reach out to the SC/team to determine an agreed upon start date of services. Usually, an initial 40-80 units should be added to the client’s plan (for initial meetings/observations and development of the initial BSP).
*The consultant will develop the BSP within 60 days of authorization (unless extenuating circumstances occur - ie. difficulty scheduling needed observations, delay in authorization, illness of consultant/client)
*Within the BSP, the frequency and duration of behavioral support will be noted. Once the BSP is received by team members, SC should begin revision to add in the additional units as recommended by the BSC and agreed upon by the team.
